The Office of the Vice-Dean, Research & Health Science Education (OVDRHSE) has received funds from the Vice-Provost, Innovations in Undergraduate Education, to support the participation of EDIIA-focused guest speakers in our basic science undergraduate programs. Funds are limited and will be allocated to departments on a first-come, first-served basis. Note: This form should be completed by the department's Undergraduate Coordinator/Associate Chair or the Course Coordinator of the course in which the speaker will be participating.
For University of Toronto students who are completing course-based research at a hospital/institute that requires them to complete certain immunizations and/or testing before they begin their work, if the required services are not already provided by the hospital itself:
Students can go to Student Health & Wellness but will require proof that the health services are required due to their program of study, i.e., the Immunization Confirmation Letter for Students.
A department contact (Chair, Associate Chair, Course Instructor or Undergraduate Administrator) should fill out and sign the form, and then the student brings a copy of the signed form to Student Health & Wellness along with the immunization/testing checklist provided by the hospital. This will allow our students to be charged reduced fees for some of the health services.
Please note that this only applies to students who are conducting research as part of their program of study towards their degree. It does NOT apply to students who are conducting paid (stipend-based) research.
